The patient is a young patient with Barlow's disease. In an endoscopic approach a Memo 4D annulus and chordae apple to posterior leaflet. Secondly the length of neo-chordae is adjusted in order to avoid anterior coaptation, hence risk of anterior leaflet's systolic motion. It shows in a very didactic way of how to move the coaptation zone as posteriorly as it is possible
